Call for speaker on integration of care - European public health conference (Milan, 16 October 2015)

EPF is looking for a patient to speak about integration of care from a patient perspective at a European public Health Conference.

What? European public health conference, session on integrated care

Organisers: EUPHA, Roche, and Università Cattolica del Sacro Cuore.

Where? Milan

When? The session on integrated care takes place on Friday, 16 October 2015

How? The costs will be covered by the conference organisers.

Speaker’s specifications

We are looking for a patient who can talk about integration of care from a patient perspective, including topics like self-management support needs, the integrated care team, the role of informal carers/family, patient empowerment, and the challenges and needs related to using new technology (such as self-monitoring, mobile apps, etc.)

The session is focused on diabetes – so it would be best if the person was a patient with diabetes, but it does not necessarily have to be if  she/he can address integration of care generally.
